Resumo: A 35-year-old male reported a 1-year history of growth in the roof of the mouth. His family history included a sister who died of cancer. On intraoral examination, a 4-cm fibrous nodule was found at the junction between the hard and soft palate, which was painless, sessile, well-defined, and reddish in color. An incisional biopsy was taken following aspiration of a whitish secretion. The diagnosis was pleomorphic adenoma. The lesion was surgically removed under general anesthesia, and after 17 years, the patient returned with another 2-cm nodule, significant bone destruction involving the nasal cavity and right maxillary sinus. An incisional biopsy was taken and a diagnosis of carcinoma ex pleomorphic adenoma was made. The patient underwent maxillectomy and prosthetic oral & maxillofacial rehabilitation with a 4-year follow-up. This case illustrates the importance of long-term follow-up of patients with a history of salivary gland tumors.
