Resumo: A 50-year-old woman with Peutz-Jeghers syndrome, diagnosed with pemphigus vulgaris (PV) in 1994 started treatment with pulses of cyclophosphamide, dapsone, azathioprine and prednisone. Currently, she is taking only prednisone every other day. She was referred to the dental service of the reference hospital for follow-up of oral lesions. Anamnesis revealed previous treatment of the lesions with tetracycline, triamcinolone acetonide, and clobetasol propionate 0.05%, with no satisfactory improvement. The intraoral physical exam revealed partial dentition, edematous and desquamative areas of the upper gingiva, biofilm accumulation, ulcerated areas, and melanotic macules in different regions of the oral mucosa. Nikolsky´s sign was positive. The proposed treatment was oral hygiene instructions, professional tooth cleaning, dexamethasone elixir prescription, corticosteroid dose adjustment, and weekly follow-up. Resistant oral lesions in PV demonstrates that the multidisciplinary approach and management are fundamental for treatment success, as it enables an individualized and efficient treatment, improving the patient´s quality of life.
