Resumo: Objective: To evaluate the occurrence of oral candidiasis in head and neck cancer patients, and to evaluate therapeutic modalities and clinical outcomes. Study design: This systematic review was registered in PROSPERO and written in accordance with the PRISMA guidelines. We performed the searches in the following databases: PubMed, Web of Science, Science Direct, Scopus, Embase, Cochrane Collaboration Library, and in gray literature. The reading and selection of these articles were performed by two reviewers. Results: Twelve articles met the established inclusion and exclusion criteria. The studies selected were published in the period between 1993-2022 and evaluated the occurrence of oral candidiasis (OC) in 629 patients, of which 67.7% were male. Oral mucositis associated with OC was observed in 89.0% of the patients, and the pseudomembranous clinical type was the most prevalent. Regarding the treatment, fluconazole was the most used (34.9%). Conclusion: Fluconazole is the mainstay treatment for OC due to its effectiveness and low toxicity reported in prior studies. However, in fluconazole refractory cases, itraconazole has proven to be effective. In addition to pharmacological treatment, non-conventional therapies have shown satisfactory results.
