Resumo: A 56-year-old woman with complaints of pain in the right side of the face, for four months. Intraoral clinical examination there was a swelling in the mouth floor right side, of hardened consistency, normal mucosa color, well-defined limits, and sessile. The occlusal radiograph of the mandible showed no radiographic images associated with the lesion. The aspiration puncture was negative. The initial diagnosis was Pleomorphic Adenoma and a malignant tumor of salivary gland, then an incisional biopsy was performed. The anatomopathological examen showed cuboid epithelial cells with basophilic nuclei and clear cytoplasm organized in islets and cords assumed tubular or ductal aspect, sometimes this duct showed basophilic material suggestive of mucus and tumor cells near nerve fillets and blood vessels, compatible with Adenoid Cystic Carcinoma. The treatment proposed by the head and neck surgeon will be surgical excision associated with chemoradiotherapy. The relevance of this case is the unusual presentation.
