Resumo: The glandular odontogenic cyst (GOC) is a rare cyst development and can manifest aggressive behavior. Man, brown, 46 years old, with increased volume in the right side of mandible. The cone-beam tomography shows a hypodense, multilocular image with defined banks in symphysis and mandible body, it presents involvement of the roots from dentals elements 32 through 46. The histological examination of the incisional biopsy confirmed GOC. The lesion has been removed for curettage and peripheral osteotomy with preservation of the teeth. After 6 years, there was a recurrence of the lesion so a conservative but similar procedure was operated. After 6 months, a lesion was noticed in the lingual region of teeth 44 and 45. Also, a new enucleation with peripheral osteotomy appeared in that area. Twice a year, the patient under goes clinical and imaging follow-up because 55% of the cases show recurrence when conservation treatment has been done.
