Título: ORAL MANIFESTATIONS OF HISTOPLASMOSIS: A CASE REPORT

Resumo: Histoplasmosis is a systemic mycosis caused by Histoplasma capsulatum, a dimorphicfungus. Its oral presentation is more common in the disseminated form of the disease. A 70-year-old male patient, smoker, with history of oropharyngeal cancer, who was referred forevaluation of painful ulceration affecting lip, for approximately 15 days. Intraoralexamination revealed ulcerated lesions located in lower lip and tongue. Clinic hypothesis wassyphilis, and incisional biopsy was performed. Histopathological analysis revealed non-caseating granulomatous inflammation composed of epithelioid macrophages. Periodic Acid-Schiff (PAS) staining demonstrated small oval yeasts. Serological tests were negative foranticardiolipin and positive for Histoplasma capsulatum (M band). Based on this, diagnosisof histoplasmosis was established. Radiographic exam no revealed pulmonary changes.Patient underwent antifungal therapy (itraconazole - 200mg/45 days). Complete remission oforal lesions at 30 days. Follow-up of 18 months without signs of recurrence of oralmanifestations.
