Resumo: A 22-year-old male brown patient, was referred to the stomatology clinic with a complaint of "swollen cheeks". Anamnesis did not reveal smoking, diabetes mellitus or systemic arterial hypertension, and he was under orthodontic treatment. Extra-oral examination showed a sessile swelling in the left cheek region, near the labial commissure, without enlarged regional lymph nodes. Intraoral examination revealed a normochromic, well-defined, encapsulated, hardened to palpation, painless nodule in the left jugal mucosa, measuring approximately 5 cm in diameter and with six months of evolution. The diagnostic hypotheses were sebaceous cyst and pleomorphic adenoma. An excisional biopsy was performed with histopathological report of Pilomatricoma. A tumor of hair follicle origin, it exhibits characteristics of differentiation of hair follicle matrix cells, with the presence of hair matrix basaloid cells, transitional cells and amorphous shadow (ghost) eosinophilic cells. Careful clinical examination and knowledge of the lesion favor accurate diagnosis and therefore appropriate treatment.
