Resumo: Lichenoid lesions may be within a wide spectrum of diseases and show different severities. Two cases of lesions suggestive of lichen planus in association with leukoplakia, with different outcomes, are described. A 59 year-old female, complaining of burning sensation in the buccal mucosa, presented white plaques with irradiated striae in the buccal mucosa bilaterally. Biopsy suggested oral leukoplakia in retromolar area and lichen planus in buccal mucosa. After a two years follow-up, criteria for proliferative verrucous leukoplakia was fulfilled. A 62 year-old female presented with white plaques and striae on the buccal mucosa and tongue, bilaterally. She took several medications, and after 5 years, there was significant reduction in reticular pattern, which was suggestive of lichenoid reaction. As the clinical and histopathological features of leukoplakia, lichen planus, lichenoid lesion and lichenoid reaction may overlap, as well as they may present concomitantly, follow-up is key for diagnosis definition.
