Título: NECROTIZING PERIODONTAL DISEASES AS A POOR PROGNOSTIC FACTOR IN PATIENTS WITH HEMATOLOGICAL MALIGNANCIES: A CASE SERIES

Resumo: Necrotizing periodontal diseases (NPD) have been described in patients with hematological malignances (HM) as an oral complication of prolonged neutropenia or as a feature of undiagnosed HM. However, we suggest NPD as an indicator of poor prognosis in HM individuals. We also describe clinical strategies for NPD diagnosis and management. Microbiological cultures were obtained from 7 patients treated at a brazilian cancer hospital. Fever was presented in 5 patients (71.4%), but only 3 were severely neutropenic (< 200 neutrophils). None presented very prolonged neutropenia (maximum of 31 days). More than one microbiological germ was detected in 4 patients (57.1%). Five patients died of infectious shock complications. NPD treatment showed better performance after the identification of germs presented in necrotic tissues and their antibiotic sensitivity. Also, periodontal tissue debridement, ultrasonic or hand instrumentation, local antibiotic irrigation and photodynamic therapy may be considered.
