Resumo: A Male patient, born in 1955, came to the clinic in 2013, reporting gum sensitivity and pain in the left mandibular permanent second molar. A clinical examination and radiographic evaluation were performed. The dentist conducted a periodontal treatment on the patient because the gums were slightly swollen and red. On periodontic reevaluation, a worsening was identified, so the patient was referred to an endodontist, who stated that the lesion was endodontic-periodontal. The lesion persisted even after periodontal and endodontic treatment, so the patient was referred to a stomatopathology specialist, who indicated the extraction of the affected tooth and biopsy of the lesion, obtaining the diagnosis of gingival squamous cell carcinoma. After the diagnosis, the patient began a two-year antineoplastic treatment. In conclusion, the diagnosis was surprising because of the clinical appearance and the type of bone loss.
