Resumo: Medication-related osteonecrosis of the jaws (MRONJ) is a severe complication of patients treated with either antiresorptive or antiangiogenic drugs. A 56-year-old woman undergoing palliative treatment for breast cancer, reported the use of acid zoledronic, trastuzumab and pertuzumab. She was referred to the dentist to evaluate a painful oral lesion that evolved over a month. Patient reported the use of lower partial denture 6 months ago. Intraoral examination revealed 4mm bone exposure in the left posterior lower alveolar ridge, no signs of infection. Panoramic radiography did not show destructive areas. Diagnosis was MRONJ stage 1. Photobiomodulation, PENTO protocol, chlorhexidine and removal of trauma were adopted as treatment. However, patient evolved with infection and increased bone exposure. Patient is under follow up; conservative management was conducted since she is ineligible for a surgical intervention due to general conditions. MRONJ treatment is a challenge, conservative approach may help increase patients' quality of life.
