Resumo: The central giant cell lesion (CLCG) is a rare, non-neoplastic bone pathology that affects gnathic bones, with a wide range of behavior and a high recurrence rate. Male patient, 16 years-old, complaining of swelling in the chin with history of facial trauma. Clinical exam presented an extra and intraoral volume increase in the anterior region of the mandible. Image exams revealed a radiolucent, unilocular image, defined margins, approximately 40 mm in diameter, with expansion of the cortical bone. An incisional biopsy was performed with a diagnosis of CLCG. Laboratory tests were performed, such as calcium, phosphorus and alkaline phosphatase dosage, and ruled out hyperparathyroidism. The treatment performed was curettage of the lesion. Due to the high recurrence rate of the lesion, periodic clinical and radiographic follow-up was carried out, and after 24 months the lesion recurred, with need of a new intervention. Close follow-up is essential for recurrence early detection.
