Resumo: The patient searched assistance in 2015 due to jaw pain and a bad smell after he had implants installed. The patient mentioned the use of bisphosphonate (zometa and zoladex) in the past for prostate cancer treatment. On physical examination, it was observed bone exposure, submandibulr swelling, trismus and purulent drainage leading to the diagnosis of bisphosphonate-related osteonecrosis. The Patient was submitted to an anterior jaw resection and placement of a reconstruction plate but during follow-ups, it evolved with new bone exposures. Multiple debridement under local anesthesia and a marginal resection under general anesthesia were performed, with no success. After a year of minimal invasive managments and a new CT exam showing new areas of necrosis, a full jaw resection and new reconstruction plate was performed, placed on the remaining jaw ramus. The patient is in the 6 years postoperative period and has evolved well so far with no signs of recurrence.
