Resumo: Polymorphous adenocarcinoma affects minor salivary glands and has low potential for metastasis. A 66-year-old woman presented a tumor lesion of 1.5 cm in the upper lip mucosa and incisional biopsy was performed to investigate. At microscopy, a non-circumscribed malignant neoplasm with invasion of neoplastic cells in the stroma, either collagenized or hyalinized, was observed. The neoplastic cells were monomorphic with a ductal phenotype, consisting of polygonal cells with eosinophilic cytoplasm, vesicular ovoid nucleus with little hyperchromatism, evident nucleoli and mucous cells. The immunohistochemical panel performed to differentiate between polymorphous adenocarcinoma and mucoepidermoid carcinoma showed immunopositivity for CK7, CK14, mammoglobin, S100, SOX10, p63 and AML negative favoring the final diagnosis of polymorphous adenocarcinoma. The patient was referred to the oncology service and is awaiting treatment initiation. It is concluded that the morphological and immunohistochemical studies were essential in the diagnosis and direction of treatment.
