Resumo: A 37-year-old female patient with a giant tumor of five years evolution located in the right mandible that produced marked facial asymmetry is presented. Clinically the tumor appeared as a large ulcerated red mass that emerged from the center of the jaw. It extended from the distal face of the canine to the ascending ramus and protruded into the mouth´s floor. The whole hemimandible was affected. The teeth involved in the lesion were loose. The patient manifested mild pain and absence of paresthesia. The computerized tomography of the jaw disclosed a large oval hypodense intraosseous tumor surrounded by a thin cortical bone with perforations. The histological study showed spindle cell proliferation into a fibro-myxoid tissue matrix with areas of collagenization. The IHC demonstrated smooth muscle actin staining positive in spindle cells. Calponin was positive focally CD34 and SOX 10 were negative. The diagnosis was myofibroblastic tumor. The treatment was hemimandilectomy.
