Resumo: Graft-versus-host disease (GVHD) is an immunological event that affects recipients of hematopoietic stem cells transplantation (HSCT), causing an exacerbated inflammatory response in the body, which can involve several organs and be lethal. A 44-year-old male was diagnosed with acute lymphoblastic leukemia and underwent two HSCTs. Twenty days after the second HSCT, the patient´s intraoral clinical picture already involved a painful atypical ulcerative lesion on the dorsum of the tongue, irregular borders, darkened pigmentation. The diagnosis of GVHD was achieved by exclusion of other lesions, ruling out oral mucositis and secondary infection. Yet, there was already a confirmed diagnosis of the disease in skin, lung and liver. Systemic and topical corticosteroids and antimicrobial photodynamic therapy were used during a long treatment. Patient has been followed up and under control for 6 months. Diagnosis was complex because the patient has many comorbidities, leading to anxieties of the patient and the multidisciplinary team.
