Resumo: Leishmaniasis, a disease caused by intracellular protozoans, rarely presents as isolated oral mucosal lesions. A 53-year-old patient presented with a 10-month history of sore throat, reporting the use of itraconazole prescribed by the otorhinolaryngologist. He has no history of skin lesions. The intraoral examination showed a painful ulcer and a cobblestone appearance in the soft palate, uvula, oropharynx and nasal mucosa. With the clinical diagnosis of paracoccidioidomycosis, an incisional biopsy was performed. The histopathological analysis revealed pseudocarcinomatous hyperplasia with granulomatous inflammation and caseous necrosis in the lamina propria. After Grocott-Gomori and Giemsa stains, no fungi or parasites were found. The diagnosis of leishmaniasis was achieved by Polymerase Chain Reaction, and the patient was referred to treatment. The broad spectrum of oral granulomatous diseases and the absence of microorganisms in histopathology, which could be influenced by the antifungal treatment before the final diagnosis, can make the diagnosis of leishmaniasis a challenge.
