Resumo: A 48 year old male patient was referred to the Stomatology clinic showing a wound on his lower lip. On clinical examination, a circumscribed ulcerated lesion was observed, with hardened edges, located on the lower lip, on the left side, measuring approximately 1.5 cm, with evolution approximately 1 month and pain presence. The patient was a smoker for 30 years, worked on sun exposition and was under medical treatment for bladder infection and prostate-related conditions. The diagnostic hypotheses were: Syphilis and Squamous Cell Carcinoma (SCC). Faced with the diagnostic hypotheses, a rapid syphilis test was performed with negative result. Specific exams as Anti FTA-ABS and VDRL were requested. The patient showed resistance in carrying out hematological exams and incisional biopsy, not understanding the case severity. The Basic Health Unit was contacted and he agreed to perform the biopsy, which resulted in CPB. The patient was referred for oncological treatment.
