Resumo: Polymorphous low-grade adenocarcinoma (PLGA) is a rare and malignant salivary gland tumor, that most frequently occurs on the minor salivary glands. Hard or soft palate are the most common locations for PLGA, followed by the upper lip and the buccal mucosa. It typically presents as a firm mass with slow growth and is occasionally associated with discomfort or bleeding, affecting mainly older female adults. A 64-year-old female presented a symptomatic mass located on the right side of the buccal mucosa, covered by normal mucosa, firm on palpation, with a gradual increase in size and pain for three months. The clinical hypothesis included minor salivary gland tumor. The patient underwent an incisional biopsy and the histopathologic analysis reported indistinct cell borders, eosinophilic cytoplasm, and peripheral cells invading the perineural and skeletal muscle. The histopathologic diagnosis was PLGA and the patient was referred to the local cancer hospital, where treatment is ongoing.
