Resumo: A 77-year-old woman presented to Stomatology Department of a hospital complaining of pain and burning tongue swelling, with 90 days of duration. The patient had a diagnosis of breast cancer in 1992, with pulmonary metastasis in 2015. Currently, she was on maintenance treatment with tamoxifen. A slight posterior enlargement indurated on palpation of the tongue was observed. Amyloidosis, sarcoidosis and metastasis were our main diagnosis hypotheses. An incisional biopsy was performed. The histopathological exam revealed an undifferentiated tumor which showed immunoreactivity for GATA-3, estrogen receptor, progesterone receptor, p16, cytokeratin AE1/AE3, and 34BE12 antibodies. The diagnosis was a breast cancer metastasis to the tongue. The patient was referred to clinical oncology. Fulvestrant was initiated, and 45Gy of intensity-modulated radiation therapy was administered. Clinicians should consider metastasis when evaluating patients with a history of malignancy who present oral nodules, regardless of the period of involvement of the primary tumor.
