Resumo: This randomized double-blind controlled trial compared the effects of photobiomodulation therapy (PBMT) and topical clonazepam for managing burning mouth syndrome (BMS). Twenty-one patients were randomly allocated to either the intervention group that underwent six PBMT sessions with an infrared low-power laser (880nm, 90mW, 6J/cm2) or the control group, which rinsed with 5ml of 0.5mg/ml clonazepam solution three times a day for 21 days. All patients were evaluated according to the IMMPACT-II recommendations for outcome assessment in pain clinical trials by a blinded researcher at the baseline, end of treatment, 30-day, and 60-day follow-up. Both treatments achieved significant symptom relief in BMS patients without a significant difference between them. PBMT provided an earlier effect that was maintained for at least 30 days, while topical clonazepam had a slightly higher long-term effect at the 60-day follow-up. Both PBMT and topical clonazepam are effective in controlling pain symptoms in BMS patients.Funding: Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - Brasil (CAPES) - Finance Code 001
