Resumo: A 44-year-old male patient sought specialized care in 2014 complaining of an asymptomatic "rough spot in the mouth", with an evolution of one year, associated with frequent occlusal trauma in the region. The patient´s past medical history included a bone marrow transplant a year before, having been diagnosed with GVHD (graft versus host disease) six months after the transplant. During the intraoral examination, a whitish plaque was seen next to tooth 37, leading to the hypothesis of leukoplakia. An incisional biopsy was performed, in which analysis showed mild epithelial dysplasia. During follow-ups, the appearance of the lesion remained unchanged until recently, when the lesion became more verrucous, and biopsy was performed again. The anatomopathological result indicated severe dysplasia of the epithelium or verrucous carcinoma, representing malignant transformation. Therefore, the patient was referred to a specialized oncology service, undergoing surgery with total removal of the lesion.
