Resumo: Squamous cell carcinoma (SCC) accounts for more than 90% of oral malignant neoplasms, especially in the tongue of men over 50 years. A 78-year-old male patient, alcoholic and smoker, first presented a painless lesion in the anterior region of gingiva that evolved to an increased painful lesion, extending from the midline to the right mandibular body after a one year history of evolution. Incisional biopsy of the lesion was performed with the hypothesis of a malignant lesion. Histological analysis revealed proliferation of islands and nests of pleomorphic epithelial cells, infiltrating the underlying connective tissue, cellular and nuclear pleomorphism, areas of keratinizing with keratin pearl formation, and moderate chronic inflammatory infiltrate. The diagnosis of well-differentiated squamous cell carcinoma was established. The patient was referred to the oncology service and is under radiotherapy treatment.
