Resumo: Subgemmal neurogenous plaques (SNPs) are subepithelial biphasic nerve plexus associated with taste buds. Appropriate knowledge of clinical and morphological aspects of SNPs can avoid misdiagnoses of neural neoplasms. Here, we present a case of a 41-year-old female that attended a stomatology service reporting intermittent stabbing pain in the posterior right border of the tongue, often while chewing, associated with loss of taste. The pain extended to other areas of the head and neck. During intraoral examination, an erythematous area near the foliate papillae was identified on the right side of the tongue. No extraoral changes nor relevant medical information were reported. Diagnostic hypotheses included tissue trauma and SNPs. An excisional biopsy was performed. Histopathology revealed a superficial neural component, parallel to the surface epithelium and a deeper portion formed by small nerve fascicles intermingled with scarce mature ganglion cells and the final diagnosis was SNPs.
