Resumo: A 78-year-old male was diagnosed with Merkel's cell carcinoma of the base of the tongue. Treatment consisted of chemotherapy (carboplatin and etoposide) and 70Gy radiotherapy. Four years later, extraction of the tooth 38 was indicated due to periodontal diseases and osteoradionecrosis was established in this region. aPDT (Antimicrobial Photodynamic Therapy) using methylene blue 0.01% and red light (660 ± 10nm; 100mW) was performed weekly for 4 weeks. Afterwards, the patient was submitted to local bone debridement and the region healed completely. After 9 months, a new exposed bone was observed in the same region. Antibiotics were prescribed and weekly applications of aPDT were performed until the complete remission of bone exposure. The present study showed a case of osteoradionecrosis which had a satisfactory control using aPDT. Such technique was well tolerated. Moreover, controlled prospective study should be performed for better understanding its efficacy in the management of osteoradionecrosis.
