Título: NODULAR MELANOMA IN THE MALAR REGION OF AN ELDERLY MEN: A CASE REPORT

Resumo: A 69-year-old man presented a dark spot on his left malar region with a history of 15 years of evolution. The lesion progressed over a period of 3 months to a blackish papule surrounded by an erythematous halo, with episodes of purulent secretion and bleeding. Upon clinical examination, a palpable lymph node was observed ipsilateral to the lesion. An excisional biopsy was performed, with a clinical suspicion of nodular melanoma. Histopathological analysis revealed cohesive aggregates and sheets of neoplastic melanocytes in the dermis with extensive pagetoid spread, presenting pigmentation in some areas. The Clark level was considered III and the surgical margins were free of neoplasia. Angiolymphatic and perineural invasion were not detected. The pathological staging was classidied to pT4a and the diagnosis of a nodular melanoma was established, based on the combination of clinical and histological findings. The patient was referred to the oncology servise for treatment.
