Resumo: A 55-year-old white skinned male patient went to the Oral Diagnosis Clinic complaining of a significant increase in his right maxilla, causing displacement of the eye globe, with history of 6-years´ duration and a marked growth in the last year. Intraoral examination showed an ulcerated mass covering the hard palate, causing pain and difficulties in speaking and eating, leading to the hypothesis of a salivary gland malignant tumor or sarcoma. An incisional biopsy was performed, which revealed a malignant salivary glandular neoplasm with a pseudocystic structure. Immunohistochemical analysis was positive for CK7, CK8, CK14, and vimentin, and negative for S-100, SMA, mamoglobin, CK20, and CDX-2. The final diagnosis was adenocarcinoma not otherwise specified (NOS), and the patient was referred to an oncologist. Adenocarcinoma NOS is a rare malignant salivary glandular neoplasm of challenging diagnosis due to the variety of histological features and growth patterns, as illustrated by this case.
