Resumo: Report 4 cases of subgemmal neurogenous plaque (SNP), their clinical and histopathological features, and treatment. There was a predilection for females with an average age of 49.5 years. Clinically, they presented as nodules or papules of varying colors ranging from normochromic, erythematous to yellowish, between 0.5 to 2cm. The patients reported painful symptoms, and in some, burning or tingling, with no identifiable etiological factor. After excisional biopsy, histopathological diagnosis was established by the presence of fusiform cells, sometimes wavy or virguliform, organized in plexuses or dispersed in the connective tissue. Some cases had taste buds, inflammatory cells and lymphoid tissue. During follow-up, they reported pain improvement and no recurrence. SNP is a structure found in the tongue with a prevalence in women over 40 years old and is associated with painful symptoms. Therefore, its recognition by clinicians and pathologists is highlighted in order to avoid overdiagnosis.
