Resumo: Plasmablastic lymphoma is a rare and aggressive malignant neoplasm. A 28-year-old HIV-positive man with detectable viral load presented an erythematous gingival swelling associated with tooth mobility located from the canine to the third molar in the lower left teeth region. An Incisional biopsy was performed, with the clinical hypothesis of Kaposi´s Sarcoma. Microscopic examination revealed a malignant neoplasm with a diffuse predominance of the epithelioid pattern, showing proliferation of aggregates of large disjointed cells. Immunohistochemistry (IHC) was positive for CD138 and Epstein-Barr virus and 95% of cells marked with Ki-67, showing intense proliferation of cells. Although, IHC was negative for CD20, CD3, CD79a, and cytokeratins (40, 48, 50, and 50.6 KD). The diagnosis of plasmablastic lymphoma was established, by the combination of morphological and immunohistochemical findings. The patient was referred to the oncology service where a 6 cycles of chemotherapy treatment (CHOP protocol) was planned.
