Resumo: Gnathic osteosarcomas are rare and agressive neoplasms. Clinically, the most common symtoms are swelling, pain, paresthesia, mobility in the teeth involved, and nasal obstruction. Herein, we present a case os osteosarcoma of the maxilla in a 28-year-old male patient, which has been evolving for 12 months after treatment beginning. Radiographically, the lesion presented an extensive hypodense image involving teeth 24 to 27, with disruption of cortical bone and osteolytic appearance resembling sunbeams. The lesion was considered unresectable, and the patient continues with excusive palliative treatment.The first treatment choice of gnathic osteosarcoma is complete surgical removal, which may be associated with chemotherapy and radiotherapy. Compared to mandibular lesions, maxillary lesions are most difficult to resect due to their anatomical location, and therefore, have a worse prognosis. Death more often results from progression of the lesion than from metastasis, and early diagnosis and treatment are essential to improve survival rates.
