Resumo: Patients admitted to Intensive Care Units (ICU) are often immunocompromised, predisposing to opportunistic infections, such as reactivation of human herpesvirus (HHV). This study reports atypical oral manifestations related to HHV in patients admitted to ICU. Case 1: female, 50 years old, hospitalized for perforating acute abdomen. Cases 2 and 3 were both male patients, 45 and 52 years old, respectively, hospitalized for COVID-19 complications. The three cases presented extensive erosive lesions in the oral and perioral mucosa, superimposed by bloody crusts. The diagnostic hypotheses were viral infection and erythema multiforme. Swabs were collected and PCR for viral research confirmed the presence of HHV, confirming the viral infection. Symptomatic control and local measures such as antimicrobial photodynamic therapy and antiviral drugs were administered. Oral herpes viruses infection in immunocompromised patients may have atypical clinical manifestations and the correct diagnosis is important for assertive treatment with prognostic implications for general conditions.
