Resumo: SARS-CoV-2 infection has been showed to be able to reactivate latent virus such as EBV, HSV and CMV. Some oral leukoplakia with proliferative behavior and hairy appearance may suggest EBV infection in a similar way to the past with hairy leukoplakia in AIDS patients. The authors will report a case of hairy leukoplakia in a patient previously infected by SARS-CoV-2 who became seriously ill and was intubated for 50 days. The patient was a white man of 64 years-old, with hairy leukoplakia in lingual gingiva witch rapidly grow in the period of 14 days. The gingival lesion appeared after COVID-19 associated with other opportunistic infections. Biopsy revealed focal malignant transformation into well differentiated squamous cell carcinoma. The hairy leukoplakia was surgically removed, and the patient remains under follow-up. This case may indicate a future tendency for other similar cases, possibly EBV positives, which should be studied with appropriate techniques.
