Resumo: A 92-year-old patient came to our Stomatology Services complaining of pain in the floor of the mouth and concerned because a previously dentist suggested a Squamous Cell Carcinoma (SCC). The ulcer was rigid, erythematous, with associated lymphadenopathy. An incisional biopsy was performed revealing a stratified squamous epithelium with hyperplasia, intense mononuclear inflammatory infiltrate, ducts with metaplasia resulting in an epidermoid patter. The diagnosis was closed as a Necrotizing Sialometaplasia (NS). Another four cases sought our service due to lesions on the hard palate that were primarily diagnosed by other clinicians as SCC. All histopathological analyzes showed findings compatible with NS. Although the most common region of NS is the hard palate, NS may occur in other regions such as the floor of the mouth. It is a relatively uncommon lesion, but still SN must be considered as a differential diagnosis. Especially in patients who are at risk, such as smokers.
