Resumo: A 49-year-old male patient presented mouth lesions associated with severe pain. Clinical examination, swollen lips and stomatitis with red granulations were observed on the lower lip, buccal mucosa, soft palate, and oropharynx. Laboratory and imaging tests were requested and together with an incisional biopsy, paracoccidioidomycosis (PCM) diagnosis was confirmed. Analysis of the panoramic radiograph showed a well defined radiolucent image with a sclerotic halo in the right mandibular ascending branch. Due to the diagnosis of PCM, patient started the antifungal therapy, and after three months and regression of the PCM lesions, he returned for intraosseous lesion treatment. The patient underwent enucleation and curettage associated with chemical cauterization using 5-fluorouracil. A diagnosis of odontogenic keratocyst (OK) was confirmed. The patient has been under follow-up for nine months without signs of recurrence. Although simultaneous, control of PCM was necessary before OK treatment.
