Resumo: A 57-year-old melanoderma woman presented a nodule lesion on the left hard palate. The lesion was single, red in color, firm consistency, sessile base, sensitivity to touch, non-infiltrative size 3.5 cm. She has a medical history of sinusitis, anxiety, depression, systemic arterial hypertension, diabetes and she reports smoking habits for thirty-five years and frequency of five cigarettes a day. Tomography and incisional biopsy were requested for the diagnostic hypothesis of pleomorphic adenoma. At microscopy, malignant neoplasm was observed with the presence of cuboidal cell ducts of eosinophilic cytoplasm, rounded nuclei, myoepithelial cells with scarce cytoplasm and hyperchromatic nuclei. The neoplasm presents cribriform arrangement and desmoplastic stroma with mononuclear inflammatory infiltrate favoring the histopathological diagnosis of high-grade adenoid cystic carcinoma. Lymphovascular and perineural invasion were not identified. Immunohistochemical analysis showed immunopositive myoepithelial cells for p63 and AML, the luminal cells was immunopositive for CK7. The patient undergoes chemotherapy without distant metastases.
