Resumo: This study reports a clinical case of cherubism with active bone lesions after puberty. Male, melanoderma, 16 years-old, with bilateral expansion in the mandible, that was firm at palpation and present for about 8 years, with a growing peak in the last 2 years. The cone-beam tomography showed multilocular radiolucent lesions in mandible bilaterally. Histopathological exam of the incisional biopsies revealed central giant cell lesions bilaterally. Histopathological and tomographic features led to the diagnosis of Cherubism grade I of Fordyce, as well as grade I/ class 5 of Motamedi and Raposo-Amaral. Blood biochemical markers showed elevated values of alkaline phosphatase, glycemia, alanine aminotransferase, triglycerides and thyrotropin, and deficiency of free thyroxine and vitamin D. Patient is under follow-up by a multidisciplinary team with Pediatric Endocrinology, Oral Pathology and Oral and Maxillofacial Surgery, currently under study to define a non-invasive treatment approach with salmon calcitonin as the first treatment option.
