Resumo: An 18-year-old male was referred to a hospital dental office 26 days after undergoing Allogeneic Hematopoietic Stem Cell Transplantation, complaining of lip pain, difficulty ingesting solid food and limited mouth opening. Clinical examination revealed dryness, edema, hemorrhagic crusts on the lip vermillion, as well as ulcers on the labial commissures. Diagnostic hypotheses were Acute Graft-Host Disease (aGVHD) and Stevens-Johnson Syndrome (SJS). The lesions progressed to necrosis on the lips and ulcers on the anterior region of the buccal mucosa, similar to SJS oral lesions and not commonly seen in aGVHD. The disease evolved with a distribution of diagnostic significance that confirmed aGVHD: maculopapular rash on the hands, feet, trunk, face, upper limbs and ears, followed by blisters formation, epidermal necrolysis, and skin exfoliation. Treatment included calcineurin inhibitors, high-dose corticosteroids, and opioid analgesics. Topical dexamethasone and photobiomodulation therapy were used for oral lesions to obtain analgesic and anti-inflammatory effects.
