Resumo: A 55-year-old-female patient diagnosed with breast carcinoma, treated with surgery, chemotherapy and radiotherapy, with two foci of bone metastasis, was referred for evaluation of numbness on the lower lip and chin. Panoramic X-ray showed a discreet poorly delimited radiolucent area in the region of the right mandibular ramus. Cone beam computed tomography demonstrated hypodense image of mandibular medullary bone with poorly defined borders and cortical bone resorption. According to the clinical symptom of paresthesia, history of metastatic breast cancer and the characteristics of the imaging exams, the main hypothesis of diagnosis was another bone metastasis and the patient was referred to her oncologist. In the follow-up, the patient presented extraoral asymmetry with increased volume in the region of the right mandibular ramus and evolution of radiolucent and hypodense areas with severe disruption of the distal cortex of the right mandibular ramus. The patient died 3 years after the first visit.
