Resumo: A 78-year-old male with a history of clear cell renal carcinoma presented to a dentistry service to address oral issues prior to chemotherapy. During clinical examination, two lesions were identified on the upper gingiva. The patient reported using a complete dental wear, which did not fit due to the lesions. The diagnostic hypothesis of reactional or metastatic lesions were considered, and biopsies of both lesions were performed. Microscopic examination revealed intense, diffuse, and infiltrative proliferation of atypical and pleomorphic cells with abundant and granular cytoplasm in a loose stroma containing necrosis, and mixed acute and chronic inflammatory infiltrate. An immunohistochemical panel was conducted with the vimentin, PAX8, renal cell carcinoma antigen, SOX10, and EMA biomarkers being positives. The findings were consistent with carcinoma of renal origin. The information was communicated to the oncologist, and the treatment was initiated. After six months, there was a regression of the oral lesions.
