Resumo: Objective: The aim of this systematic review was to analyzes of palatal recurrent pleomorphic adenomas to determine their clinical outcome. Study Design: A systematic review was conducted in diverse databases according to PRISMA guidelines and registered at PROSPERO. After applied of screening and eligibility criteria in 610 articles, 13 studies were selected. The risk of bias was assessed using JBI for use in Systematic Reviews tools. Results: The population included 18 patients with recurrent pleomorphic adenoma in palate, predominantly female, mean age of 48.9 years-old, and with average time of first recurrence of 15.9 years-old. Malignant transformation of tumor was detected in 22.2% of patients. The time of tumor recurrence was associated with increased odds ratio in 3.545 (p=0.302) after 5 years and in 13.000 (p=0.028) after 10 years of follow-up. Conclusion: We concluded that the rate of malignant transformation of palatal recurrent pleomorphic adenoma is considerable, and a long follow-up of patients is recommended. Funding: CAPES - code 001
