Resumo: Paracoccidioidomycosis is a systemic fungal infection described in 1908 by Adolf Lutz in Brazil. Here, we report a case of a 50-year-old male patient, who presented ulcerated lesions in the oral cavity for five years, crusty lesions in perioral and glabella regions, local lymphadenopathy, trismus, dry cough, and recent weight loss of 13kg. Incisional biopsy revealed a chronic granulomatous inflammatory process with multinucleated giant cells and numerous fungus yeast. The patient was referred to a hospital, and diagnosed with multifocal paracoccidioidomycosis involving oral mucosa, skin, lungs, and adrenal glands. He was treated with liposomal amphotericin B and hydrocortisone for 20 days. After discharge, he has been treated with hydrocortisone and Bactrim, showing regression of lesions and weight gain of more than 14kg in the last ten months, and is still being monitored. This case highlights the role of the dentist in diagnosing systemic diseases avoiding a delay in prompt treatment.
