Resumo: A 65-year-old black skinned male patient went to the Oral Diagnosis Clinic complaining of a maxillary lesion with 13-years´ duration. Intraoral examination showed a slight 3-cm vestibular swelling on the right maxilla, covered by normal oral mucosa. The cone beam tomography revealed a multilocular radiolucent lesion compromising the maxilla bilaterally. An incisional biopsy with the hypothesis of odontogenic myxoma was performed and the histological examination exhibited vascular spaces of different diameters associated to hemorrhagic areas in a dense connective tissue with the diagnosis of intraosseous hemangioma. The patient was referred for oral rehabilitation and remains under regular follow up. Intraosseous hemangiomas in jaw bones are uncommon and use to be diagnosed posterior mandible of young women. The biopsy was essential for the diagnosis of this case given the rarity of this lesion, especially if considered to the location, age and gender of the patient.
