Resumo: A 52-year-old female patient with melanoderm sought routine dental care. During a periapical radiographic exam for diagnostic complementation, a radiolucent image with radiopaque foci inside was observed located in the apical region of teeth 32, 31, 41, and 42. Cone beam computed tomography showed a mixed density image with precise limits and discrete expansion of the bone cortices. Based on clinical and radiological information, the diagnostic hypothesis was ossifying fibroma or periapical cemento-osseous dysplasia. Incisional biopsy with subsequent histopathological analysis confirmed the diagnosis of periapical cemento-osseous dysplasia according to the anatomopathological report. Postoperative recovery was uneventful. At the postoperative follow-up, pulpal necrosis was diagnosed in teeth 41 and 42 due to the rupture of the neurovascular bundle in the post-curettage region. After six months of monitoring, a discrete neoformation was observed in the biopsied region.
