Resumo: The aim was to report a case of an atypical oral manifestation of Cytomegalovirus (CMV) infection in a 48-year-old female patient admitted to the Intensive Care Unit (ICU). The patient was hospitalized due to acute respiratory failure (pulmonary pneumocytosis), requiring orotracheal intubation. Infection by the Human Immunodeficiency Virus (HIV) was detected during hospitalization allowing a diagnosis of acquired immunodeficiency syndrome. Intraoral evaluation showed multiple ulcers on the lips and tongue, with an erythematous background, accompanied by whitish mucous patches. The first clinical hypothesis was syphilis. The dental care team performed an incisional biopsy and Antimicrobial Photodynamic Therapy. Serological tests for syphilis (VDRL and FTA-abs) were negative and positive for CMV. Histopathological analysis was compatible with herpetic infection. The patient was treated with antiretroviral drugs, and the lesions were completely healed. After clinical improvement, the patient was discharged from the ICU.
