Resumo: Paracoccidioidomycosis is a systemic fungal disease most frequently in South America that has a predilection for men because hormonal factors. An 76-year-old heavy smoker woman presented to oral medicine service with ulcerations on the oral mucosa and pulmonary involvement without diagnosis, investigating a possibility of tuberculosis, lung carcinoma or metastasis. The oral lesions were firm, leukoerythroplasic, had a moriform surface and affected the alveolar ridge, oral and buccal mucosa and floor of the mouth. Due to the clinical appearance and pulmonary complaint, the main diagnostic hypothesis was paracoccidioidomycosis. It was realized an incisional biopsy in lower lip and the histopathological diagnosis confirmed the infection. The patient was referred for treatment. Although uncommon, the manifestation of this disease in a female is probably correlated with the loss of hormonal protection due to the patient's age. This report highlights the importance of the dentist in the diagnosis of systemic diseases.
