Resumo: A 69-year-old male patient referred for a complaint of a tumor in the mandible. During physical examination he reported a history of abscess in the anterior region of the mandible. Clinical examination, an increase in intraoral volume was observed, with no pain, foul odor, and purulent discharge. Imaging exams revealed an extensive radiolucent area in the body of the mandible crossing the midline, involving teeth 32 to 46, well-defined, extending from the right ramus of the mandible to the chin, and with expansion of the bone cortices. An incisional biopsy was performed, and a diagnosis of odontogenic keratocyst was made. The treatment performed was enucleation and curettage of the lesion associated with chemical cauterization with 5-fluorouracil, to prevent receurrence. The patient has been under 12 months of follow-up without complaints or signs of recurrence. Correct diagnosis is important so additional measures can be performed to avoid recurrence of the cyst.
