Resumo: Different malignant neoplasms can affect the oral cavity. This study investigated the prevalence of oral cancer, its subtypes, possible associations, and risk factors in cases referred to one of the reference services in Oral Pathology in Brazil. Thirty-eight years of archived cases were evaluated. Based on the records, the histopathological subtypes were classified, and possible associations were analyzed. The prevalence of oral cancer was 3.35% (712 cases of 21.276 biopsied oral). The four most common malignant lesions, in decreasing order, were: squamous cell carcinoma-SCC (594; 83.71%), mucoepidermoid carcinoma-SCC (25; 3.51%), adenoid cystic carcinoma-CAC (22 ; 3.09%) and polymorphic-ACP adenocarcinoma (16; 2.25%). There was a statistically association between males and SCC (p < 0.001) and between females and SCC (p < 0.001), CAC (p < 0.001) and ACP (p=0.01). Smoking was associated with SCC (p < 0.001). In 83.3% (p < 0.001) of patients with MEC, the average age was then 58 years.The results show that SCC is the main oral cancer, being the smoking an important risk factor. The knowledge of its prevalence is important to establish strategies that encourage early diagnosis.
