Resumo: Body Dysmorphism Disorder is a psychiatric condition associated with dissatisfaction with one´s own body image, which leads patients to come by various aesthetics surgical procedures. This case portrays a 41-year-old woman that underwent more than thirty plastic surgeries and got multiple grafts with polymethylmethacrylate (PMMA) and industrial silicone, dating back at least 10 years, in the bilateral jugal, nasal and zygomatic prominence regions, chin and mandible angle. PMMA is a biomaterial used in surgical rehabilitation. Despite the benefits, it may present toxicity due to the residual monomer. The patient showed intraoral granulomas in the bilateral lower vestibular region, two fistulas in the right pre-auricular region, sensation of facial paralysis (House-Brackmann 3) with severe muscle impairment. The biomaterial was surgically removed under anesthesia, with the support of ultrasound to identify the material, which was forwarded for histopathological analysis. The patient was followed up postoperatively for six months.
