Resumo: Leishmaniasis is a parasitic infection caused by several Leishmania species that might exhibit oral lesions. Mucocutaneous leishmaniasis affecting the maxillofacial region is related to significant morbidity rates. This study aims to report the case of a 57-year-old male who attended the Cubatão Stomatology Service complaining of "mouth and nose ulcers". On physical examination, multiple granulomatous ulcers affecting palate, gingiva, and nose were observed. An incisional biopsy was performed, and histopathological characteristics were suggestive of leishmaniasis. Immunohistochemistry for Leishmania confirmed the diagnosis of mucocutaneous leishmaniasis with oral manifestation. The patient was referred to infectology but refused treatment and clinical follow-up. Several reasons caused delay to achieve final diagnosis, and treatment neglect, as well as the sociocultural status of the patient, led to an epidemiologic challenge, turning the patient into a disease vector in his resident region.
