Resumo: The objective of this report is to present a case of odontogenic myxoma, in which conservative surgical treatment was performed with preservation of the integrity of the mandibular nerve (CNV3), in a 23-year-old female patient. Computed tomography showed an extensive hypodense image in the left body and branch of the mandible. The diagnosis of myxoma was confirmed by incisional biopsy. The treatment consisted of enucleation of the lesion, extraction of teeth 36, 37, 38, peripheral ostectomy in the inside bone portion, with cryotherapy complementation and application of 10% formaldehyde. The cavity was filled with autologous blood and a polypropylene membrane was used for alveolar occlusion. After 7 years of follow-up, the patient is now rehabilitated with implant prosthodontics and no recurrence has been observed. This case shows that conservative surgical removal of odontogenic myxoma in the mandible is an effective option of treatment.
